1. Original Official Receipt of Current Payment
2. Original Certificate of Registration (CR)
3. PNP-TMG Clearance for Change of Engine Transaction
4. Motor Vehicle Inspection Report (Stencil)
5. Original Sales Invoice with certification
6. Certificate of Stock Report (CSR) from LTO Central Office Registration Section, duly evaluated and received/stamped by the Operations Division of LTO Central Office or its Regional Office
7. Appropriate Insurance Certificate of Cover (COC) or endorsement from the Insurance company stating the new engine
8. Taxpayer Identification Number (TIN) of new owner
9. In the absence of 5, affidavit of lost original sales invoice and non-registration with warranty to be executed by the registered owner
10. Affidavit of Change Engine
11. Secretary?s Certificate of Board Resolution if the registered name is a corporation, stating therein authority of the signatory to sign in behalf of the corporation
12. File application ONLY at LTO where the Certificate of Registration was issued
