DUPLICATE CERTIFICATE OF REGISTRATION WITH ENCUMBRANCE
1. Original Official Receipt of Current Payment 2. Photocopy of Certificate of Registration with Encumbrance (CRE) 3. Affidavit of lost Certificate of Registration, indicating lost CR serial number in affidavit 4. Motor Vehicle Inspection Report (Stencil) 5. At least two (2) valid identification cards from Registered owner photocopy to be submitted at LTO after presentation of Original ID?s. 6. Certification from the financing company/bank, indicating: 1. That the Original CRE is no longer in their custody 2. Conformity allowing Duplicate CRE. Note: Certification must be duly notarized 7. Original Copy of the Release of Chattel Mortgage duly stamped and received by the Register of Deeds, if encumbrance is already settled/paid 8. Original Official Receipt of Register of Deeds 9. TIN of registered owner 10. File application ONLY at LTO where the CRE was issued
