1. Affidavit of Lost Plates, as provided for/in prescribed format by the LTO
2. Certified Photocopy of OR/CR
3. Motor Vehicle Inspection Report from MVIS (either from South MVIS Domestic Road, Pasay City or North MVIS East Ave., Quezon City)
5. File application ONLY at LTO where the License Plates was issued
6. TIN of registered owner
